Start by creating a rounded rectangle using rounded rectangular shape tool.
Double click the layer to open layer style window. Apply settings as shown.

Reduce the fill value to 0%. We'll call this layer as "image place holder" layer.
Create a rectangle using rectangular shape tool. Enable "subtract from shape area" at the top bar. Create a rounded rectangle using rounded rectangle shape tool.
Double click the layer to open layer style window. Apply settings as shown.
Create corners for rest of the sides.
Create two rectangles using rectangular shape tool. Apply some bevel&emboss layer style.
Create a circle using elliptical shape tool.
Double click the layer to open layer style window. Apply settings as shown.
Similarly create bolt heads for rest of the corners.
Create a rectangle using rectangular shape tool. Apply pattern overlay layer style and use same pattern we used in the beginning.
Ctrl+Click to get the selection. Contract the selection by 2 pixels. Create a new layer. Apply Black to white gradient. Change layer mode to screen for this layer.
Duplicate this rectangle layer. place on the other side. Fix a couple of bolts for each rectangle.
Insert image of your choice. Move this layer above the "image place holder" layer in layer order in layer palette.
