Start by creating a new file of header size. Create a rectangle covering the whole document using rectangular shape tool.
Double click the layer to open layer style window. Apply settings as shown.
Create a rounded rectangle using rounded rectangle shape tool. Apply stroke layer style.
Create another rounded rectangle using rounded rectangle shape tool. Apply stroke layer style.
Create a rectangle using rectangular shape tool. Fill it with lighter shade of green.
Create some "Ornament 8" shapes using custom shape tool.
Select and merge all the ornament shape layers. Press Ctrl+Alt+G to create a clipping mask.
Create a rectangle using rectangular shape tool.
Double click the layer to open layer style window. Apply settings as shown.
Insert a relevant image. Move this layer above the black rectangular layer in layer order in layer palette. Press Ctrl+Alt+G to create a clipping mask.
Type link names as shown in the image below. Create diamond shapes using custom shape tool and place before the links.
